[PATCH] Kill dead visualActivate and all associated paraphenilia

Hi... the KIconEffect::visualActivate method hasn't done anything
since the initial KDE4 port, 2+ years ago (and in KDE3 it drew andugly
border effect and made things slower). It's also not used anywhere in
the main modules (there is one use in KEG). The attached kills this method
and all the extra machinery associated with it: kglobalsettings stuff, kcm
stuff, etc. OK for monday?
