Removal of menu icons (Re: Defaulting to "Icons on Buttons")

I guess in the end, we would be happy to make small icons if we clear
out some of the repetitive and sometimes confusing entries in the menus
(I know, this started out about icons on the buttons). This would
seriously make creating the icons easier.
