khtml (win32) patch for review

- compile fixes for msvc

- compiler warnings removed for msvc

- KHTMLWalletQueue moved to khtml_wallet_p.h

- in render_object.h:
   added void operator delete(void* ptr, RenderArena* renderArena);
   to avoid warning about lacking operator delete. Should this operator's
   implementation be empty? There are other places where the same operater
   can be added.
