On OpenBSD 3.6 and KDE 3.2.3 printed PDF files are missing the top 
3 line on every page using the following printcap entry:

# APS1_BEGIN:printer1
# - don't delete start label for apsfilter printer1
# - no other printer defines between BEGIN and END LABEL
# APS1_END - don't delete this
