[PATCH] improved kurlbar

a problem that has been identified on the kde-usability list is that the 
kurlbar speedbar thing on the left doesn't scale very well with a number of 
entries as it results in a vertical scrollbar quite quickly. by default we 
load as many as 7 entries in that speedbar, so this is a common situation! 

a solution is to switch to small icons, but the small icons display doesn't 
look very good currently. the attached patches make the small icons larger 
(22px), increases the spacing between entries, and introduces squeezing of 
the labels. the attached patches also change the default for the filedialog 
speedbar to small icons. i'd like to see this happen for 3.2 as it really 
does improve the look and usability of this often used dialog.
