bugzilla query page

Hi,

someone modified the bugzilla query page on the server without committing 
it to cvs. That's not such a good idea, it confuses people :-) As the page 
had also some bugs (missing anchor, some borders missing, not valid XHTML) 
I restored the version from cvs. Modified version is still on the server 
as form.html.tmpl.bak2.
