ugh, that took me long. :}

the patch should:
- make things work under any circumstances
- make the --with options override cached values. fwiw, i think this
  should apply to all configure switches ...
- barf if something absent is requested
it also touches the shadow stuff, as i was already at it.

please report success or failure, particularily on systems not based on
linuxpam 0.74.
