IBM developed a 100% "Self-Voicing-Kit", which could be configured to be a
screen reader (among other things).  It worked directly with the Java
Accessibility API, and only worked within a specific Java VM.  It was never
released as a product, though quite a few folks have played with evaluation
copies.
