Gettext's msgmerge tool takes care of that, not KBabel. When you get a 
new POT file, run msgmerge to update the PO file. New strings will show 
up as untranslated strings in the PO file, and changed strings will 
show up as fuzzies.

In KDE, msgmerge is run regularly, so that the translators don't have to 
deal with it themselves.
