The information is always stored in the database. Whether they are written to 
the metadata depends on the options in the metadata settings. Whether sidecars 
are used also depends on whether they are activated. Note the advanced 
metadata settings, here you can specify for some (e.g. tags, comments, rating 
etc.) in which EXIF/IPTC/XMP are written. You can set it individually.

Since digiKam-8.0.0 it is possible to write metadata with ExifTool because our 
internally used Exiv2 is limited.
So if you have activated writing with ExifTool, you can write directly to the 
metadata for all formats that ExifTool supports, including HEIC/HEIF images.
