It's always better to backup all.

But typically Sqlite DB is not changed and it use it as well.

The Mysql Database change, due of lots of bugs. It's safe to restart it
from scratch.

There is a bug about general settings (digikamrc files). The place is
different to store KDE application rc files and digiKam ignore for the
moment the older one.
