I already modified this code and checked every single pointer and data
structure I could think of 

(
d->m_plugin,  d->m_plugin->actions(), 
d->m_plugin->actions().isEmpty(), 
d->m_plugin->actions()[0] == 0, 
d->m_plugin->actions()[0]->icon().isNull()
), 

but still it crashes. I don't know why...?
