Hi! I really love the examples like "bad example" and "good example". It is a 
great idea!!!

Reproducibility field is also an interesting information. I only hope that 
reporters will not be scared of too many fields to fill.

Anyway I mainly agree with what Dario answered you.

I want to add a small note: I'd prefer to have the component list a bit 
longer. At least with 8-9 items displayed.

The "additional information" description maybe is a bit too long: we have 
already read and write a lot, the last description will be probably skipped 
:-)

I'd like to have the possibility to add attachments from this form too.
