OT: A short survey

Hey All,

This is very off topic, but I need a wide range of samples for this
survey. It is part of a semester long HCI usability project.  There
are about 15 questions, mostly multiple choice. It shouldn't take
longer than a minute to do, and I would really appreciate it (and
indirectly so would my grade).
