The second problem seems to be fixed with KDE Frameworks 5.47.0, and the first
one is probably WONTFIX. 

a) Not a lot of users actually use the kio-man slave to view manpages
b) There are pretty much zero manpages which do use pictures (correct me if I
am wrong)

I'll therefore close this bug report, but please file a new one against
frameworks-kio if you do think that this is a feature which is important enough
to spend time on. Thanks!
