I was able to recreate some problematic scenario.

If you modify some menu entry in kmenuedit (which wasn't modified before) then
a .desktop file is created in $HOME/.local..., so it's possible to change it
later through kmenuedit or app launcher--> edit app. All changes are visible in
app launcher and krunner then.

But, if you try to edit some menu entry in App launcher--> edit app, then just
a symlink to /usr/.../...desktop is created in $home/.local/ so it can't be
modified from anywhere then (kmenuedit saves changes without warning, but
nothing really happens).
The only way to fix that is to remove symlink from .local/share/applications
and modify it in kmenuedit first.
