Are you using the Breeze Dark icon theme with the Breeze Dark Plasma theme?.
Breeze uses the gray 16 px folders, Breeze Dark uses light gray 16 px folders.
That icon is either folder.svg (places/user-folders-small/) or
inode-directory.svg (mimetypes/small-file-types) the latter being a link to the
former, and folder.svg in Breeze Dark is, as I mentioned earlier light gray.
