This is because creating a new file is internally simply a "copy from
template", so the file replace dialog appears when trying to create the same
file again.

There are actually two minor bugs reported here.

The modification date is taken from the template file, but it could display the
current date instead (this would also get rid of "Destination is more recent"
warnings).

The text for the "Overwrite" button's tool tip is not context sensitive; the
comment about you will be prompted again" does indeed not apply to single-file
copies, but to recursive copies.
