Hmm.. could you please make the experiment: save the correct config, then
change "Active=True" to "Active=False" and test if the further settings made
are saved correctly? The question is whether krandr can't work with some old
config (it changes from time to time) or it just reads and writes the config
incorrectly (even new one). If you could try it it will be very helpful. And I
also have a look at load/save code in krandr.

As to StartupCommand, it is executed not by krandr but in startkde script, it's
written directly from config and executed immediately. So you should see the
proper display layout just at the beginning of KDE log in. Krandr just saves
the StartupCommand item in the config according to the settings you provide.
But when you press apply, afaik it really don't execute StartupCommand but
apply settings another way.
