Can you confirm that this only happens when you are using the Oxygen or the
Ozone window decoration (and therefore they are first shown when switching to
the "Windows" module in the Appearance page on System Settings?

Here using:
Qt: 4.5.0 + qt-copy-patches-936035
KDE: 4.2.69 (KDE 4.2.69 (KDE 4.3 >= 20090406))
kdelibs svn rev. 951854 / kdebase svn rev. 951854
on ArchLinux i686 - Kernel 2.6.28.8
Intel GMA x3100 (xf86-video-intel 2.6.3)
xorg-server 1.6.1

I can only reproduce this with those two style, so I guess it's a Oxygen code
bug.
