The icon in the top right corner of the code import, code generator and class
window is not shown. Instead an empty icon is displayed.

Reproducible: Always

Steps to Reproduce:
1. open umbrello
2. open class wizard


Actual Results:  
An empty icon is shown in the top right corner, also in the other mentioned
dialogs.

Expected Results:  
The requested icon should be displayed.
