Patch to fix a Phonon-GStreamer StreamReader bug

Thanks to Laurent Pinchart, who made me look into StreamReader again, attached 
is a patch that should fix StreamReader a bit.

The QByteArray.mid change should make the buffer move forward as many bytes as 
were actually read. In addition there's a fixme comment.
