<p>This removes ExpressionAst::belongsToCall, because it's always equal to ExpressionAst::parent or null.</p>

<p>TODO: UseBuilder::visitName() needs tidying, and the new argument to functionDeclarationForCalledDeclaration() - can I rename that to something shorter, please?! - could be used elsewhere; possibly to make ExpressionBuilder::visitCall() simpler.<br />
