RFC: Refactor CD player logic from kscd

Hi all,

I consider the refactoring of kscd.cpp's CD player logic now complete, with 
the help of some great feedback.

Now I'd like to look at the settings. It seems a bit of a mess with 
duplicate/overlapping values in the cdaudio kioslave, kscd, kaudiocreator, 
and maybe amaroK too. Roughly speaking I propose:

1. There should be a common definition of the (default) CD playback device. At 
this point, I propose not to worry about multiple devices. [shared by kscd, 
kaudiocreator at least].

2. There should be a default set of setting for encoder codec settings [shared 
by cdaudio kioslave and kaudiocreator at least].

3. There should be a default set of setting for encoder output paths [shared 
by cdaudio kioslave, kaudiocreator and visible in amaroK at least].

Quite honestly, I'd like some feedback on this idea to avoid wasted work.
