Sorry, that is the extent of my experience with KDE and menus.
I have never paid attention to the output when running it, but this may 
be part of the problem

You might try to mv ~/.config ~/.config.bu or something and try again 
(to build a new ksycoca?)
and then you have the .config.bu to help restore your personal settings

Also try a different user (create a new one or even try root) and see if 
things are normal. If they are
normal, you'll at least have something to go on. compare the .config 
files and the .kde files to see
what's different.
